Download, print, complete and mail to the A&FSC.

Download Consent for Release of Information
Download, print, complete and mail to the A&FSC. This allows us to obtain any information from previous agencies and/or attorneys you may have worked with in the past.

Download Adoption Disclosure
Downoad, print and return a signed copy to the A&FSC.

Download Affidavit of Good Moral Character
Download, print, complete, NOTARIZE and mail. Be sure to print off two copies of this affidavit-one for you and one for your partner.

Download Addendum of Affidavit of Good Moral Character
Download, print, complete, NOTARIZE and mail. Once again, be sure to print off two copies of the addendum-one for you and one for your partner.

Download VECHS Waiver Agreement & Statement
Download, print, sign and return to our office for each adult member in the home. This allows us to review your fingerprint results.

The Family Support Center was established in 1992 and is a Licensed Child Placing Agency. Our hope is to share God’s love for each of our birth parents through our genuine care for them, and our non-judgmental and unconditional acceptance of them. We pray that, through our words and deeds, each family will come to know their intrinsic value and worth in God’s eyes.
